War Ravaged Iraqis Traveled to India for Quality Healthcare

Years of war have literally destroyed Iraqi’s once world-renowned healthcare system. The Iraqi healthcare system used to be considered one of the best in the region, but today, millions of Iraqis suffer from respiratory infections, formerly preventable diseases such as measles, and malnutrition. The WHO (World Health Organization), UNICEF and USAID have joined hands and efforts to provide essential services for many Iraqis, but continuing conflict has reduced the effectiveness and ability of such organizations to provide quality and all-around care for Iraqi citizens.

Dire Straits for Iraqis
The basics are missing from Iraqi’s healthcare facilities and hospitals these days, from vaccinations to cardiac defibrillators. Meningitis, dysentery, and malaria have become the norm. Hospital labs can’t perform tests, and lack of drugs and basic supplies severely hamper physician’s efforts to cure patients. Medicine, syringes and IV fluids are often only available through the black market.

Missing drugs, ill-equipped doctors, and sometimes infested clinics and hospitals are seriously hampering healthcare efforts in Iraq, and patients are suffering. Corruption and incompetence is replacing what used to be known as one of the most efficient, state-of-the-art, and highly educated healthcare providers in this area of the world. Money earmarked for medical drugs, equipment and clinics goes missing, and even formally world-renowned facilities and ERs in Baghdad are suffering the consequences.

Because of the dire situation in Iraq, millions of Iraqis are scrambling and saving to travel to foreign destinations for quality and affordable medical care. Intracytoplasmic sperm injection (ICSI)

Understanding ICSI for Infertility Treatments

ICSI stands for intracytoplasmic sperm injection - a process that addresses male infertility. This type of procedure is known as an Assisted Reproductive Technology Generally, the procedure is used during IVF (in vitro fertilization) treatments by injecting sperm into a mature egg for development. Following the fertilization procedure, the egg is placed into the mother’s womb or fallopian tube for implantation.

ICSI is a popular treatment for men whose sperm count is too low or inadequate to ensure fertilization of eggs. The process utilizes specialized equipment and tools, but is an effective treatment for many couples dealing with infertility issues due to a variety of causes.

Who Benefits from ICSI Treatments?
In some cases, medical practitioners or clinics will suggest ICSI treatments for couples dealing with low sperm counts or inability to fertilize the egg. It’s an ideal solution for couples that are not interested in donor sperm. In most cases ICSI treatments are geared toward those with:

  • Sperm concentrations measuring less than 15-20 million per milliliter
  • Sperm mobility is less than 35%
  • Poor sperm quality
  • Any couple engaging in IVF (In Vitro Fertilization)

Common ICSI Procedure Overview
Eggs are collected from the potential mother’s fallopian tubes or ovary. Using a high-powered magnification device, the doctor will use an injection pipet to deposit one sperm into the egg, which is held in place with a glass tool called a holding pipet. The egg or eggs are then left overnight in a laboratory and checked the following day. After the eggs have been checked for successful fertilization, they will be left to develop for several more days. Then, two to four fertilized eggs are placed in the uterus for implantation.

Other or additional eggs may be frozen at this point in time in the event that more than one attempt or ICSI cycle is necessary for growth and development of the egg once it has been implanted in the uterus. These procedures are performed on an outpatient basis and necessitate only a short recovery time.

Today, nearly half of IVF procedures utilize ICSI technology, with fertilization success about 70% to 85% using this method, which usually produces roughly 25% to 30% successful birthrates. However, couples are advised not to wait too long to benefit from such procedures. For women over 35 years of age, more embryos may have to be transferred than others, which also increases the risk of multiple births.

What is Depression?
According to World Health Organization (WHO) estimates, by 2020, depression will become the second-leading cause of disability and death throughout the entire world.  About one million people commit suicide annually, and people suffering from depression make up between 45 and 60% of suicides.  This figure is becoming comparable to infectious and cardiovascular diseases.

A person with depression has a 35 times greater chance to end his life through suicide than do those not suffering from depression.  50% of those with endogenous depression and 20% of those with psychogenic depression attempt suicide.  One out of every six commits suicide.  According to research conducted in the USA, people susceptible to depression are two times more likely to die from other illnesses.

Depression often develops among family members of a patient already suffering from depression.  Approximately 20% of relatives of depressed people suffer from depression, while relatives of healthy people become sick in 7% of cases.

Risk of depression among single and divorced people is 2-4 times higher than among people with families.  Also, single and divorced men are at a higher risk than single and divorced women.  Even if only spouse suffers from depression, divorce is 10 times more likely than among healthy families.

50% of depression sufferers do not even seek medical help, and only 25-30% of the others make it to the psychiatrist.

Stained Teeth: There is a Cure

Stained teeth are a very common dental problem that makes a lot of people to feel uncomfortable when they have to talk or smile, due to the color of their teeth. However stained teeth depending on their cause have a number of treatments.

Types of Tooth Discoloration:

Extrinsic Tooth Discoloration

  • Colored organic compounds from some foods and drinks – tea, coffee, red wine, blueberries, curry and other – are absorbed on teeth enamel, resulting in yellow teeth.
  • Other foods that contain much chlorophyll may result in green stains.
  • Common dental restorative materials, as the amalgam in silver-colored fillings, may turn adjacent areas of teeth black or gray.
  • Prescribed antiseptic mouthwashes containing chlorhexidine are associated with causing yellow-brown stains on teeth. Repeated use of stannous fluoride can result in light brown or yellowish teeth stains. Iodine-containing solution medications may also cause similar stains.
  • Metallic stains can be caused by metallic salts, such as in iron supplements.
  • Other habits as smoking or chewing tobacco are also responsible for stained teeth, creating brown to black stains on the tooth surface.
  • Accumulation of dental plaque and tartar may alter the natural appearance of the tooth and lead to yellow teeth.

Intrinsic Tooth Discoloration

  • Systemic disorders (as alkaptonuria, erythropoietic porphyria, hyperbilirubinemia etc.) are also causes of stained teeth. Enamel hypoplasia causes tooth discoloration and pitting of the enamel. In rare cases, children with dentinogenesis imperfecta are born with gray, amber or purple discolorations.
  • In fluorosis, chalky white spots or brown marks on teeth are caused by excessive fluoride intake. (tooth whitening does not actually remove the white spots but lightens the background so they are less noticeable).

APPLICATIONS WHICH INCREASE CHANCES FOR SUCCESS
What kind of new developments are underway in test-tube baby applications? The head of Acıbadem Kadıköy Hospital Test-Tube Baby Centre, Associate Professor L. Cem Demirel and Associate Professor Şahin Zeteroğlu inform us as follows:”Infertility treatment places significant emotional, physical and financial burden on the couples. In the event of repeated failures in test-tube baby attempts, this also has had an impact on the physicians in charge of the treatment of such couples. This necessitates the application of secondary approaches to shed light on the reasons of failure and ensure more successful outcomes. Here, the failure is either due to the quality of the embryo, a group of cells fertilised in the laboratory and placed in the mother’s womb which are in the process of dividing up, or to the inability of the mother’s womb to provide a sustainable environment for the embryos placed in it. If repeated inferior embryo quality is identified and observed as the reason for repeated failures in test-tube baby attempts, at Acıbadem Test-Tube Baby Centre we either subject the family to what is called an endometrial co-culture or to natural cycle IVF application. Endometrial co-culture is the name given to the practise of collecting tissue cells from the mother’s womb called endometrium, which line the inside of the womb. These cells are then passed through passages in the laboratory and the embryo is developed from these cells prior to its transfer to the mother-to-be. In natural cycle IVF, the eggs of the mother-to-be are collected by either pharmacological stimulation, no stimulation or minimal stimulation. In the event of failure with these methods, we may sometimes recommend egg donation.” In the meantime, it is important to identify the role of the sperm and exclude its underlying impact on test-tube baby failures. Dr.Demirel points out that in their unit they also perform the analysis of sperm cells, the male reproductive cells. Dr.Demirel says that they specifically check the DNA breakage ratio of the sperm cells which demonstrate the damage ratio of the DNA that forms the genetic structure of the sperm cells. He indicates that in this way, it possible to take the necessary measures in the presence of high DNA damage.

National Heart Centre Singapore Unveils New Techniques For Heart

Heart patients with small, difficult to treat vessels requiring stenting procedures may soon have another viable option to treat coronary artery disease. Known as the CardioMind SparrowTM Drug-Eluting Coronary Stent System, this stent is 70% smaller in diameter than any other currently approved stent and targets at blood vessels smaller than 2.75mm in diameter.

The Sparrow Stent System is an improved version of the current stents in the market. Conventional balloon-expandable stents travel over guidewires to the lesion in the artery. The Sparrow Stent is incorporated into the distal end of the guidewire itself, therefore having the same outer diameter and flexibility as the guidewire. The stent travels as part of the guidewire to the site of the lesion. The cardiologist then releases the stent and allows it to self-expand and support the vessel wall. The tightness of fit of conventional stents, as well as their inflexibility and the forces delivered by the balloon expansion, may contribute to increased vessel trauma, which in turn leads to less favourable long-term results. The Sparrow Stent also offers more flexibility than the current stents, making it especially adaptable to the treatment of small, tortuous blood vessels often associated with diabetes, which represent 40% of the patients who require a stenting procedure. Over time, the polymer of the Sparrow Stent will gradually biodegrade away and return the stent to a bare metal state. Biodegradable polymers for drug-eluting coronary stents are thought to contribute to a reduction in late stent thrombosis rate.

The centre estimates that 10% – 30% of the patients requiring percutaneous coronary interventions (PCI) may benefit from this revolutionary stent. The Sparrow Stent is currently being evaluated in a clinical trial known as the CARE II Study.

This multi-site randomised trial will test both the drug-eluting and bare-metal versions of the Sparrow, plus a competitive stent in 220 patients. The National Heart Centre Singapore is currently the only Asian site to have started in the CARE II Study. Four patients have been treated since the trial started here in September 2008.

“All four patients had uncomplicated procedural and hospital outcomes. I see a great potential for this stent since many of our diabetic patients have small vessels.” said Associate Professor Koh Tian Hai, Medical Director, National Heart Centre Singapore and also the Principal Investigator of CARE II in Singapore.
