Thalassemia, an inherited, genetic blood disorder, is a chronic debilitating disease which can be lethal. Patients must struggle through their life in order to fight this severe disease. To live longer, they need constant treatment involving regular blood transfusions as well as regular clinical visits. Repeated blood transfusions can also cause problems of iron overload which can damage many organs such as liver, heart, skin and pancrease. LASIK is the most advanced laser vision correction surgery today and is capable of correcting nearsightedness, farsightedness, and astigmatism. The ability of the modern excimer laser to remove tissue with sub-micron precision has created a sudden interest and over a million people will have LASIK performed this year.
Over half of the world’s population is affected by these traits and are reliant on spectacles or contact lenses to view the world clearly. Glasses can be bothersome and uncomfortable while contact lenses are equally problematic. They dry out your eyes, are sometimes painful, and starve your eyes of precious oxygen. With contact lenses there is also the constant and sight-threatening risk of infection (about 1 in 2000 per year) which increases with poor hygiene. With improved technology and greater experience, LASIK surgery has now become far safer than contact lens wear. When performed by a corneal expert, the one-time sterile procedure has been shown to be much healthier for the eyes than decades of uncomfortable contact lens use.
During LASIK, the surgeon creates a corneal “flap” with a cutting instrument, called a microkeratome. The flap is lifted and the underlying cornea is reshaped using a “cold” excimer laser. The flap can then be carefully replaced and the tissue heals itself within a couple days without sutures, adopting the new shape and providing improved vision within weeks and sometimes overnight. Overseas patients can stay in Bangkok just five days for the whole process, including the preoperative exam, the surgery itself, the one day and the 1 week postoperative exam.

Drink only “bottled” water or carbonated beverages and wash hands frequently. Other options include medication for self-treatment of diarrhea.
- “Outdoor” diseases – Bush walking, trekking, hiking, swimming, camping are activities that are commonly enjoyed in this part of the world. However, again, precautions need to be taken. Rabies is very common in our region, and if there is a risk of contact with animals, rabies vaccination prior to travel is strongly advised. Further, all contact with animals, including dogs, cats, monkeys, bats should be avoided. There are numerous tick-borne diseases that can be contracted from bush walking; hence it is highly advisable to wear appropriate shoes at all times. In certain regions, swimming in unchlorinated water may put you at risk for contracting parasitic diseases. Also, there are known to be outbreaks of communicable diseases associated with swimming and expedition in certain areas.
- “Other” risks - These include sexually transmitted diseases and body fluid transmissible diseases such as HIV, hepatitis b, and hepatitis c. Always maintain protected sexual contact at all times. If engaging in activities requiring body piercing or needles, do not share needles and be sure that sterile needles are used at all times.

Screening Examinations

These atheromatous plaques narrow the arteries but as long as the heart muscles are getting a reasonable amount of oxygen from the blood supply, there will be no symptoms. If, however, the arteries narrow to a critical level then the blood flow will be insufficient to support the heart muscles cells and a group of them, lacking oxygen, may die. The patient may have a heart attack; some may not survive.

Stem cells are undifferentiated or partially differentiated cells. Therefore they are capable of dividing, renewing themselves and also giving rise to specialized cell types, a phenomenon known as plasticity or transdifferentiation. Stem cells can be obtained from many sources such as embryo, fetal, umbilical cord blood, bone marrow or peripheral blood. The adult stem cells are the stem cells found in the mature tissue. Some scientists use the term “somatic stem cell” instead of adult stem cell. Most fetal and adult stem cells are considered to be multipotent and therefore capable of producing a small range of differentiated cell lineages appropriate to their location. Some adult stem cells or progenitor cells with the least differentiation potential such as skeletal myoblasts, angiogenic precursor cells (APCs) or epidermal stem cells in the basal layer of the skin are designated as unipotent.
The Bangkok Heart Hospital is now treating the patients with heart failure and patients with damaged heart muscle from blockage of the coronary artery using angiogenic precursor cells produced from the patient’s own peripheral blood. The treatment is in collaboration with Theravitae Ltd. and the University of Pittsburgh. The patient will have his/her medical history and physical examinations taken as usual. A number of blood tests will be screened for the blood concentration, infection (e.g. hepatitis, HIV, bacteremia) and general health (e.g. liver and kidney function). The Six-Minute method will be used used to assess the physical status of the patient before and after surgery. In addition, the magnetic resonance imaging of the heart will be used to assess the heart muscle viability. If the patients are eligible for the therapy, a 250 cc of autologous blood will be collected from the patients and it will be then sent to the laboratory for cell selection and expansion. These processes take about 1 week. Then the final cell product will be sent back to the Bangkok Heart Hospital for the administration.
The cardiac surgeon will make a small incision at the left side of the chest wall. The thoracoscopic camera will be placed though a small hole into the thoracic cavity to assist with the procedure. Basically the cells will be injected directly into the cardiac muscle where the damaged heart muscle is located by the magnetic resonance imaging of the heart.
What are the results of the treatment?
Up to date, the cell therapy from various kinds of cell origins and from various types of administrations demonstrates that this cell therapy is feasible and improves the function of the damaged heart muscle. These APCs are capable of generating new vessels and perhaps new heart muscles. This new area in medicine and rapid evolving field is in need of more comprehensive research.
